Chandra Education Activities

We ask that you take the time to fill out this evaluation form for each investigation, performance task, activity or demonstration that you use. There is also a general section at the end for comments not tied to a specific activity. Your evaluations will be used to update, revise and fine-tune the materials on this site.

The pdf form may be submitted electronically to our email account. Or, you may fill it in, print it out and mail it to us at:
